5. Appointments, Scheduling, and Cancellations
Our website may allow you to request, confirm, or cancel appointments using online forms or links to Curve Dental scheduling tools. Appointment requests submitted through the website are not confirmed until you receive confirmation from our office.
We understand that schedules change. However, when an appointment is reserved for you, that time is set aside for your care. Late cancellations and missed appointments limit our ability to offer that time to other patients.
The practice reserves the right to charge a cancellation fee of up to two hundred dollars (200 USD) for appointments that are cancelled with insufficient notice or for missed appointments, consistent with our in office cancellation policy as communicated to you by our team.
6. Payments and PCI DSS Responsibilities
If online payments are offered through this website, all payment transactions are processed by Curve Dental Payments, a third party payment processor that maintains Payment Card Industry Data Security Standard (PCI DSS) compliance.
- The practice does not store, handle, or manage credit card numbers or payment card data on its own servers.
- Payment information is submitted by the user only within secure Curve Dental Payments systems or their hosted payment interface.
- Curve Dental Payments is solely responsible for the processing and security of cardholder data.

The practice reserves the right to refuse or cancel transactions where appropriate, including in cases of suspected fraud or misuse.
